The forms below are required for each of your clients who have booked a trip (whether or not they express interest in purchasing the insurance at this time). This form is crucial to making sure that the commission is appointed to the correct agent.

To ensure accurate quoting and proper agent credit, it’s essential to choose the correct form and provide complete details. Using the wrong form or entering partial trip info can cause delays or errors.
✅ Which Form Should I Use?

| A Canadian resident with a Canadian address | Canadian New Trip Booking Form |
⚠️ Top 5 Mistakes to Avoid
- Using the wrong form (e.g. Payee vs. New Trip)
- Entering only the balance due instead of full trip cost
- Omitting parts of the trip (e.g. cruise + land)
- Inconsistent dates or costs vs. the original Tully booking
- Canadian citizens using U.S. forms

Please select the appropriate form:
NEW TRIP BOOKING FORM – This form is for new bookings. Everyone on one form lives in the same household and pays for their own policy.
PAYEE BOOKING FORM (PAID BY SOMEONE OTHER THAN THE TRAVELER) – This form is for groups that live in separate households, but one person is paying for everyone’s insurance, or an individual household is being purchased by someone other than the travelers.
TOP UP/ ADD ON FORM – This form is to increase the trip cost to an existing policy.
DATE CHANGE ONLY FORM – This form is for existing policies that need a date change only, no increase to the trip cost.
CANADIAN NEW TRIP BOOKING FORM – This form is for Canadian citizens with a Canadian address.
